First Office Visit

Your initial appointment at our office is a vital step on your journey to Bariatric Surgery.  At this visit you can expect to meet our entire team.

When you arrive you will be greeted by our front desk team who will sign you in and prepare you to meet our clinical team members.  Please remember to bring your referral and the patient forms which you can download from the Pre-Op Forms section of our website.

You will first meet with Juan Carlos Cabrera, Dr. De la Cruz' Certified Surgical Assistant who is present at all surgeries.  He will perform a complete history and physical and go over your past medical history.

You will then meet with Dr. De la Cruz-Muñoz .  Together, you will go over your past medical history and decide if bariatric surgery is right for you.   Dr. De la Cruz will also determine what medical work-up you will be required to complete before your surgery.
 

Lastly, you will meet with Melissa Cuesta, Bariatric Program Coordinator who will guide you through all the requirements prior to surgery. She will give you step by step detail regarding medical tests, specialist appointments, insurance requirements and schedule you for surgery.
